Healthy Living (CDSMP) in Apache Junction, Sept
CDSMP is the English version of the Chronic Disease Self-Management Program developed by Dr. Kate Lorig at the Stanford Patient Education Research Center. CDSMP is the core program of the Chronic Disease Self-Management Education (CDSME) programs. CDSMP is a copyrighted program that requires organizations and leaders to operate under a license. In Arizona, CDSMP is known as Healthy Living (CDSMP).
All attendees of this training will need to have an established agreement in place to provide licensing prior to attending the training. If you are unsure of your current license status, please contact the Arizona Living Well Institute at email@example.com.
CDSMP workshops require two (2) facilitators, so organizations are encouraged to sponsor two (2) members to attend the training. CDSMP Leaders are encouraged to deliver one (1) CDSMP workshop within six (6) months of their training, and are required to facilitate one (1) workshop annually to maintain their Leader status. CDSMP workshops should enroll 12-18 participants per workshop.
DATE AND TIME:The training will be held at Empowerment Systems, 2066 W Apache Trail Suite 116, Apache Junction, AZ 85120 at the following dates and times:
Monday, September 18 - 9:00am - 4:30pm
Tuesday, September 19 - 9:00am-4:30pm
Monday, September 25 - 9:00am-4:30pm
Tuesday, September 26 - 9:00am-4:30pm
Attendance is REQUIRED all 4 days.
COST: $300 per person. You will be invoiced via email.Checks should be made to Empowerment Systems, Inc. If you choose to pay by check there will be NO fees charged. You can also pay by check or by credit card using Square. If you wish to pay by credit card via Square, a 4% tax rate will be charged. Please call 480-367-6937 x 121 to pay by credit card.
PAYMENT AND REFUND POLICY: Payment is due within 7 days of registration. Full refunds will be administered with cancellation before 1 week prior to start date. Any cancellation requests made between 1 week before the start date of the training and the first day of the training, will be charged a $50 administrative fee with the remainder refunded. If cancellation occurs after the first day of the training, or the participant is a no show, no refund will be issued.
Trainees are responsible for meals, travel, and lodging expenses. All materials for the training are provided (textbook, manual, and CD). Light snacks are provided, but lunch is not!

Any organization delivering a Stanford program must operate under a Stanford license. Any organization receiving training must have a license prior to the training. Please provide us with the NAME of your current licence organization. If you do not know, please contact your organization's CDSME coordinator.
